29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy a) Family Dollar Store #3572 / 101 Diamond Street, Titusville, PA: On or about December 10, 2015 and times prior thereto, an electrical switch plate was missing and exposed live electrical wires. The missing faceplate was on the North West wall at the entrance to the stock room. b) Family Dollar Store #3572 / 101 Diamond Street, Titusville, PA: On or about December 10, 2015 and times prior thereto, an electrical switch plate was missing and exposed live electrical wires. The missing faceplate was on the North East wall to the right of the exit door in the stock room. c) Family Dollar Store #3572 / 101 Diamond Street, Titusville, PA: On or about December 10, 2015 and times prior thereto, an electrical switch plate was missing and exposed live electrical wires. The missing faceplate was on the North East wall to the left of the exit door in the stock room.

The Family Dollar was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent Standard, 29 CFR 1910.303(g)(1)(ii), which was contained in OSHA inspection number 1056989, citation number 1, item number 1 and was affirmed as a final order on October 20, 2015, with respect to a workplace located at 8971 Halls Ferry Road, Saint Louis, Missouri.
