29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  a) Assembly Area; The employer failed to verify that the required workplace hazard assessment has been performed through a written certification.  Employees are exposed to struck-by hazards while assembling straining machines.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a) Assembly Area: The employer did not certify that all employees were trained and evaluated prior to operating powered industrial trucks.  Employees in the Assembly Area operated a Hyster forklift (Model S35XM, SN C010H0) and Mitsubishi forklift (Model FGC25K, SN AF82D01064) to move equipment out of the way in order to assembly the straining machine.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Assembly Area; The employer had not established or implemented a written Hazard Communication program.  Employees were exposed to chemicals such as, but not limited to propane gas (flammable, compressed gas) for use with forklifts, oxygen (oxidizing compressed gas) for use during welding, and acetylene (flammable compressed gas) for use during welding.  Exposure to propane and acetylene gases could result in burns from a fire or broken bones from impact of compressed gas canister.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Assembly Area; Employees were required to work with chemicals such as, but not limited to propane gas (flammable, compressed gas) for use with forklifts, oxygen (oxidizing compressed gas) for use during welding, and acetylene (flammable compressed gas) for use during welding.  The employer did not provide training specific to the hazards of chemicals used at the establishment to all employees working with hazardous chemicals.  Exposure to propane and acetylene gases could result in burns from a fire or broken bones from impact of compressed gas canister.
