29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) At the jobsite, on the work platform supported by the Gradall Rough Terrain Forklift, on or about 12/8/15: An employee was wearing a personal fall arrest harness and was attached to a lifeline that was connected to the work platform, however the lifeline was a full coil of line that would not have provided any protection from falls from the platform or from the mansard roof the worker was climbing onto to install corrugated sheet products.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a) At the jobsite, accessing the work platform on the Gradall Rough Terrain Forklift, on or about 12/8/15: An 8 foot step ladder was placed to access and egress the work platform that was approximately 9 feet from ground, causing the user to use the top level of the ladder as a step.
