29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:      (a) On or about December 7th, 2015, at a worksite located at 401 W. Maple Ave. in Independence, MO, an employee was exposed to potential injuries from unexpected scaffold movements in that, prior to the start of work, an inspection of the scaffold for visible defects was not performed.  The scaffold was placed in service and allowed to be used when an inspection for visible defects would have revealed that certain wheel and caster lock mechanisms were inoperative.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
