Employee(s) used the lathe on a daily basis to polish shafts or stems and make making new shafts for pumps.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: On or about December 11, 2015, at Industrial & Marine Valve, LLC in the machine shop, the employer failed to ensure that employees were protected from flying debris while using a Mazak lathe. The employee used an unguarded lathe without the use of safety glasses.
