29 CFR 1926.404(b)(1)(ii): Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.  Workers were engaged in commercial construction activities installing a roof vent on an existing steep sloped roof structure. Worker(s) utilized a circular saw to cut an opening on the roof peak, exposing worker(s) to potential electrocution and secondary fall hazards in that the electrical service was not protected with a ground-fault circuit interrupter and an assured equipment grounding conductor program was not being utilized.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   Workers were engaged in commercial construction activities installing a roof vent on an existing structure. Workers were exposed to potential fall hazards between 14.5 and 23.5 feet in that a proper fall protection system was not utilized.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   Workers were engaged in commercial construction activities installing a roof vent on an existing structure. Workers were exposed to potential fall hazards between 14.5 and 23.5 feet in that the workers were not trained in proper fall protection criteria.
