29 CFR 1910.213(c)(1): Circular hand-fed ripsaws were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: (a) Scene Shop - Employees used a Rockwell Unisaw circular table saw to rip wood used for building scenery. The moveable Excalibur over arm blade guard did not get used on this hand-fed circular saw, exposing employees to getting caught-in and struck-by the rotating saw blade, as occurred on or about December 8, 2015, when an employee suffered amputation injuries to the index and middle fingers and thumb of the left hand. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.213(c)(2): Hand-fed circular ripsaws were not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator: (a) Scene Shop - Employees used a Rockwell Unisaw table saw to rip wood used for building scenery. This hand-fed circular saw did not have a spreader to keep the wood from squeezing the saw blade, exposing employees who operated the saw to the hazard of getting struck-by wood kicked back by the saw, on or about December 8, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
