Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  a) Jobsite, 550 Mifflin Avenue, Wilkinsburg, PA:  On or about December 11, 2015, an employee was working in an unprotected excavation with the spoil pile located right along the edge of the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):    a) Jobsite, 550 Mifflin Avenue, Wilkinsburg, PA:  On or about December 11, 2015, an employee was exposed to cave-in hazards while backfilling inside an excavation that ranged from 5 feet deep to 7 feet deep at the ladder.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Jobsite, 550 Mifflin Avenue, Wilkinsburg, PA:  On or about December 11, 2015, an employee was using one section of an extension ladder to get into and out of an excavation that extended approximately 18 inches above the ground surface.
