29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about December 11, 2016, at the cross section of Orice Road and Roth Road in Gonzales, LA; the employer did not ensure that the employee(s) harnesses were attached to the life lines while working on a roof edge at a height greater than six (6) feet to ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about December 11, 2015, at the cross section of Orice Road and Roth Road; the employer did not have a portable ladder extended at least three (3) feet above the landing for the employee(s) to safely access and exit the roof.
