29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury: a) Pillar Elementary School: On December 8, 2015 and at times prior thereto, instructions and guidance were not provided on safe work practices while performing work, including but not limited to seal locking or "crimping" activities on the deck. An employee did not recognize or acknowledge the unsafe condition of walking/working backwards to an unprotected edge, nor were actions taken to minimize or eliminate the hazard. Note) Abatement certification and documentation are required for this item.
