29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury:    a) Pillar Elementary School: On December 8, 2015 and at times prior thereto, instructions and guidance were not provided on safe work practices while performing work, including but not limited to seal locking or "crimping" activities on the deck.  An employee did not recognize or acknowledge the unsafe condition of walking/working backwards to an unprotected edge, nor were actions taken to minimize or eliminate the hazard.           Note) Abatement certification and documentation are required for this item.

29 CFR 1926.760(d)(3): Perimeter safety cables did not meet the criteria for guardrail systems in CFR 1926.502 (see Appendix G):  a) Pillar Falls jobsite:  On October 9, 2015 and at times prior thereto the perimeter cabling that was in place on the west side of the first floor, was not flagged with high visibility material.   Note) Abatement certification IS required for this item.
