29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    At a commercial construction site, an employee in an elevated aerial lift was not protected from a fall hazard of approximately 16 feet by the use of a full body harness and lanyard attached to the basket.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    At a commercial construction site, employees were exposed to an unprotected fall hazard while performing welding activities at elevated heights more than 15 feet:    a.  on the roof area, two employees were working without fall protection lanyards being attached and anchored exposing them to a fall hazard of approximately 27 feet to the ground below.    b.  on the second level, an employee was working without fall protection lanyards being attached and anchored exposing him to a fall hazard of approximately 16 feet to the ground below.
