29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) Facility    The employer did not implement the written respiratory protection program for employees required to wear full face, negative pressure respirators.  The fit test records did not include the type of fit test performed, the make, model, style and size of respirator tested, and the results of the fit test.  Medical evaluations were not performed for the type of respirator actually used.     Violation observed on or about 02/17/16.
