29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about December 10, 2015, at the jobsite located at 2200 Glen Avenue, Salina, KS 67402:  Employees engaged in masonry activities from a tubular welded frame scaffold were exposed to fall hazards of up to 20 feet from the working surface to the ground below in that employees had not been trained in the recognition and avoidance of hazards associated with the use of scaffolding.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards:  On or about December 10, 2015, at the jobsite located at 2200 Glen Avenue, Salina, KS 67402:  Employees engaged in masonry activities, using scaffold ladders to access and descend the working levels, were exposed to fall hazards of approximately 12 feet from the ladder to the ground below in that ladder training was not provided.

The in-patient hospitalizations were not reported to OSHA within 24 hours.
