29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Each employee observed working on multi-story, multi-family residential roofs, did not have restraints attached to a rope grab device or a vertical lifeline that was attached to an anchor on the building, exposing them to fall hazards greater than 30 feet to the ground below.    In lieu of:    29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    Each employee observed working on the multi-story, multi-family residential roof did not have personal fall arrest systems rigged to prevent a fall greater than six feet when exposed to fall hazards 30 feet off the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     Employees were ascending and descending from the rooftop on a Werner extension ladder which did not extend at least three feet above the rooftop landing, which exposed employees to fall hazards.
