Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to fall and fire hazards: (a) On or about November 23, 2015 for an employee who suffered fractures and lacerations from a fall while escaping from a snubbing basket during a blowout without an adequate individual escape line approximately four miles south of County Road 53 in Watford City, North Dakota. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install an individual escape line for each employee working in the snubbing basket in accordance with the manufacturers instructions.
