29 CFR 1904.31(b)(2): The employer did not record injuries and illnesses for employees obtained from a temporary help service, employee leasing service, or personnel supply service, and were supervised by the employer on a day-to-day basis: At the Cornfields, Inc. facility - On or about December 14, 2015, the following recordable injuries and illnesses to temporary workers being supervised by Cornfields, Inc. were not recorded on the Cornfields, Inc. OSHA Form 300, Log of Work-Related Injuries and Illnesses, for calendar year 2015. a) On or about November 5, 2015, a recordable injury occurred when an employee of Unistaff, Inc. received second degree burns on the left side of his face and ear when he slipped and dropped a pot of "slurry" on the floor and his face fell on it, resulting in 25 days of lost time. b) On or about November 18, 2015, a recordable injury occurred when an employee of Unistaff Inc. received a first degree burn to the left side of his face when lifting a kettle with a co-worker and the co-worker let go of the hot kettle causing it to strike the employee's face, resulting in 1 day of restricted duty. c) On or about November 23, 2015, a recordable injury occurred when an employee of Unistaff, Inc. received an injury to their right shin when a trash can fell over and struck the employee's shin, resulting in 9 days of restricted duty. d) On or about December 2, 2015, a recordable injury occurred when an employee of Unistaff, Inc. slipped on a wet floor striking their head on a machine causing a laceration that required four staples to close, resulting in medical treatment beyond first aid.
