29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. Worksite located at 120 Hickory, Omaha, NE: The employer failed to report the hospitalization of an employee as required. On November 9, 2015, an employee was hospitalized for three days as a result of an on the job injury. Abatement certification is required for this violation.
