OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to a struck-by hazard due to the lack of adequate lockout tagout procedures.  On or about September 3, 2015, west side of Black Lake, Hackberry, LA:  Employees were exposed to a struck-by hazard while performing lockout tagout operations on gas well D-18 Stab 5 without adequate lockout tagout procedures.  While opening a valve to bleed off pressure from a 4 inch gas lift line, the 2 inch pipe riser that the valve was connected to erupted resulting in the loss of the employee's right leg.
