29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:    (a)  Boulder Natural Meats at 5340 N Franklin Street, Denver, Co  80216:  On and before November 10, 2015, the employer did not adequately guard the Ulma packaging machine to prevent the operator from having any part of his/her body in the danger zone during the operating cycle.  The metal guard in place failed to prevent operators from getting their fingers caught in the operation of the piston when the machine was in operation.  The plastic covers were not affixed to the machine or interlocked.  Condition exposed operators to a point-of-operation hazard.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Boulder Natural Meats at 5340 N Franklin Street, Denver, Co  80216:  On and before November 10, 2015, the employer did not develop, document, and utilize machine-specific lockout tag out procedures for machines with multiple energy sources, such as, but not limited to, the Ulma packaging machine.  The lockout tag procedures for the Ulma packaging machine failed to specify how to lock out the electrical, pneumatic, and water sources of energy.  The employer failed to ensure the lockout tag out procedures for the Ulma packaging machine were being utilized during maintenance activities.  Condition resulted in a maintenance employee experienced a fingertip amputation when his finger was caught in the operation of the piston.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a) Boulder Natural Meats at 5340 N Franklin Street, Denver, Co  80216:  On and before November 10, 2015 the employer did not conduct periodic inspections of the energy control procedures at least annually for machines where maintenance activities were involved.  The employer did not ensure the requirements of this standard were being followed for machines such as, but not limited to, the Ulma packaging machine.   Condition resulted in exposure to amputation hazards for maintenance personnel.

This training was not provided for machines involving maintenance activities such as, but not limited to, the Ulma packaging machine.  Condition resulted in exposure to amputation hazards.
