29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours. On August 3, 2015 at the worksite located at 2513 Atlanta Street in Blytheville, Arkansas, an employee sustained an injury resulting in inpatient hospitalization. The employer failed to report the incident to OSHA within the twenty-four hour time period as required.
