29 CFR 1926.1101(e)(2): Signs were not provided and displayed in accordance with 29 CFR 1926.1101(k)(7):    (a)     On or about December 2, 2015, Class I work such as the removal of asbestos thermal system insulation from the pipes in the warehouse and Class III work such as the encapsulation of the asbestos thermal system insulation of pipes in the warehouse was done and there were no signs posted to demarcate the regulated area in order to minimize the number of warehouse workers in the area and to protect warehouse workers outside the area from asbestos.

29 CFR 1926.1101(f)(2)(i): The employer with a work operation covered by the asbestos standard did not ensure that a "competent person" conducted an exposure assessment immediately before or at the initiation of the operation to ascertain expected exposures during that operation or workplace:    (a)     On or about December 2, 2015, Class I work such as the removal of asbestos thermal system insulation from the pipes in the warehouse and Class III work such as the encapsulation of the asbestos thermal system insulation of pipes in the warehouse was done and there had been no determination of employee exposure made from breathing zone air samples that were representative of the 8-hour time weighted average and 30-minute short term exposures of each employee.

29 CFR 1926.1101(h)(3)(iv)(A): The employer did not provide employees with a tight-fitting powered air-purifying respirator operated in the pressure-demand mode and equipped with either HEPA egress cartridges or an auxiliary positive-pressure self-contained breathing apparatus, whenever the employees were in regulated area performing Class I asbestos work for which a negative exposure assessment was not available and the exposure assessment indicated that the exposure level will be at or below 1 fiber per cubic centimeter as an 8-hour time weighted average:    (a)     On or about December 2, 2015, Class I work such as the removal of asbestos thermal system insulation from the pipes in the warehouse and Class III work such as the encapsulation of the asbestos thermal system insulation of pipes in the warehouse was done and the asbestos workers used non-powered, negative pressure air-purifying half-face respirators.

29 CFR 1926.1101(j)(1)(i): The employer did not establish a decontamination area, consisting of an equipment room, shower area, and clean room in series, adjacent and connected to the regulated area:  Note:  This requirement applies to Class I operations involving over 25 linear or 10 square feet of thermal system insulation or surfacing containing asbestos-containing material or presumed asbestos-containing material.    (a)     On or about December 2, 2015, Class I work such as the removal of approximately 200 linear feet of asbestos thermal system insulation from the pipes in the warehouse was done and there had been no three stage decontamination area established.

29 CFR 1926.1101(m)(1)(i)(A): The employer did not institute a medical surveillance program for all employees who for a combined total of 30 or more days per year were engaged in Class I, II and III work or were exposed at or above a permissible exposure limit:  Note:  Any day in which a worker engaged in Class II or Class III operations or a combination thereof on intact material for one hour or less (taking into account the entire time spent on the removal operation, including cleanup) and, while doing so, adheres fully to the work practices specified in this standard, shall not be counted:      (a)     On or about December 2, 2015, Class I work such as the removal of asbestos thermal system insulation from the pipes in the warehouse and Class III work such as the encapsulation of the asbestos thermal system insulation of pipes in the warehouse was done and there was no medical surveillance program implemented that included medical examinations, physician consultations, providing information to the physician, and obtaining the physicians written opinion.
