29 CFR 1926.250(a)(1): Materials stored in tiers were not stacked, racked, blocked, interlocked, or otherwise secured to prevent sliding, falling or collapse: a) Workplace at Paramus Mall Paramus, NJ: A helper sustained trauma to the head when the rope holding a 300 pound sign to a crane mounted truck snapped causing the sign to shift and fell on his head. The sign was not secured to the truck, on or about 12/14/15.
