29 CFR 1910.29(a)(4)(ii):  All scaffold casters shall be provided with a positive wheel and/or swivel lock to prevent movement. Ladder stands shall have at least two (2) of the four (4) casters and shall be of the swivel type.    (a) Chocolate Refining Department-Buhler 1800 Refiner: Mobile ladder stand used to access upper areas of the chocolate calendar mill refiners did not have wheel casters that locked the casters in place and prevented movement. The front three casters had locking mechanisms that were broke and did not allow the casters to be locked and prevent the mobile ladder stand from moving, exposing employees to a fall.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    (a) Employees are exposed to oils, lubricants, sanitation and cleaning chemicals, coca and other materials used in the manufacture of chocolate products and performance of maintenance work activities. Training was not provided regarding the new Global Harmonization (GHS) label elements (product identifier, signal word, pictogram, hazard statement(s), and precautionary requirements) and new safety data sheet (SDS) format.
