29 CFR 1910.26(c)(2)(iv): Ladders must be maintained in good usable condition at all times. At this establishment an A-frame metal ladder utilized for various tasks had a cracked step, exposing employees to sharp edges and decreasing the structural integrity of the ladder. The ladder was not taken out of service. In the Alternative: OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that step ladders were not free of defects: At this establishment an A-frame fiberglass ladder utilized for various tasks had a cracked step, exposing employee to sharp edges and decreasing the structural integrity of the ladder. The ladder was not taken out of service.

29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: At the PE Grinder area, employees having to re-align various sizes of polyurethane pipes did not have safe access to reach the shredder area, thus exposing employees to a fall hazard to the concrete below.
