29 CFR 1926.1412(f)(7): The employer did not document, maintain, or retain for a minimum of 12 months the crane items checked, the results of the inspection, the name and signature of the person conducting the inspection, and the date of the inspection. (a) Jobsite - On or about December 15, 2015 employees used a Grove All Terrain RT-745 crane to lift a lowboy trailer section. There was no documentation of an annual comprehensive crane inspection by a qualified person to release the crane for operation.

29 CFR 1926.1416(d)(3)(i): The crane was not equipped with a functioning anti-two block device nor was another temporary means of marking the cable to give sufficient warning to the operator along with having a spotter to while extending the boom. (a) Jobsite - On or about December 15, 2015 a Grove LT-745 All Terrain crane used by employees did not have a functioning anti-two block device.

29 CFR 1926.1417(c)(1): The procedures applicable to the operation of the equipment, including rated capacities (load charts), recommended operating speeds, special hazard warnings, instructions, and operator's manual, must be readily available in the cab at all times for use by the operator. (a) Jobsite - On or about December 15, 2015 employees were using a Grove All Terrain RT-745 crane to lift a lowboy trailer section. The operators's manual was not in the cab of the crane.
