29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c):    (a) on December 10, 2015, at the jobsite located along Sens Road in La Porte, Texas, employees were conducting work within an unprotected trench with Type B soil.  The trench was over five feet in depth.
