29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roofs edge: (a) Worksite - Roof: On or about 12/16/2015, employees were exposed to fall hazards while working on a flat commercial roof 26 to 41 feet above the lower level, where the stanchions and warning lines were set up approximately four feet from the roof's edge.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface: (a) Worksite - Roof: On or about 12/16/2015, employees were exposed to fall hazards while working on a flat commercial roof 26 to 41 feet above the lower level, where the warning lines sagged below 34 inches in height from the roof's surface.
