29 CFR 1926.403(b)(1)(iii): The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. On or about December 8, 2015 the employer was using an extension cord that was damaged. A portion of the cord's sleeve was missing and the insulation on the conductor was cut. This condition exposed employees to hazards associated with electrical shock.
