29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about December 15, 2015, on the north side of the residential house, a foreman installing asphalt shingles to a roof while using a pneumatic nail gun did not wear approved eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris. b. On or about December 15, 2015, on the south side of the residential house, an employee installing asphalt shingles to a roof while using a pneumatic nail gun did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. On or about December 15, 2015, on the north side of the residential house, an employee working on a framed structure was not protected from a 25 feet fall hazard by the use of a personal fall protection system. b. On or about December 15, 2015, on the south side of the residential house, an employee working on a framed structure was not protected from a 25 feet fall hazard by the use of a personal fall protection system. CFH Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 107711, citation number 01, item number 03 and was affirmed as a final order on December 3, 2014, with respect to a workplace located at 158 Blue Crab Loop East, Seacrest, FL 32413.
