29 CFR 1926.416(a)(1): The employer permitted employees to work in such proximity to any part of an electric power circuit that the employees could contact the electric power circuit in the course of work, and the employees were not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:   a) Job site, at/near 72 South 24th Street, Pittsburgh, PA: On or about December 15, 2015, during roofing activities on a multi-unit residential type building, the employer did not ensure that employees were protected from electrical shock hazards in that items such as ropes from fall protection devices and collapsed rain gutters were allowed to contact, rest on and abrade the service drops from the electric power lines on the street to the home.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage and sharp corners and projections were not avoided:    Instances including, but not limited to the following:  a) Job site, at/near 72 South 24th Street, Pittsburgh, PA: On or about December 16, 2015, during roofing activities on a multi-unit residential type building, the employer did not ensure that extension cords were properly utilized. Cords were not protected from damaged in that they were knotted up and tied together, and at least one cord had a cut on the outer jacket, and cords were allowed to run around abrasive/sharp surfaces such as house foundations or roof/gutter edges and were exposed to cut metal pieces falling on them.
