29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Employees operating the Wieger shear were exposed to amputation hazards and employees were not protected from contact with the hold downs and the point of operation was not guarded to prevent employees from reaching under the improperly adjusted guard.
