OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to hazards created by tangled wires while operating Wire Annealing Drawing machine: BUILDING 1, WIRE ANNEALING DEPARTMENT: Neither the No. 300 Wire Annealing Drawing machine was equipped with all necessary safety devices, controls or other systems, nor the operators were provided with appropriate means, methods and/or specific training and instructions to protect the operators from the hazards created by tangled wires while operating the machine. On/or about December 6, 2015, an operator who was operating the above machine was sustained a 2-finger amputation of the left hand and a nerve damage on the left elbow while trying to manually cut the tangled wire while the machine was in operation.
