29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: Worksite located at 23886 Race Track Rd. Spiro, OK 74959. On or about December 7, 2015, employees working from the suspended scaffolding were permitted to work in proximity to power distribution lines without de-energizing and grounding the circuits or other protective means. The employees were exposed to the hazard of electrical shock.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): Worksite located at 23886 Race Track Rd. Spiro, OK 74959. On or about December 7, 2015, employees were working from suspended scaffolding with conductive material that could have come closer than 10 feet to uninsulated power lines with a voltage of less than 50 kv. Employees were exposed to the hazard of electrical shock.
