29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) On or about December 4, 2015, the employer did not ensure employees were protected from fall hazards while using a ladder to access a roof, which was not extended at least three feet above the eaves.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)  On or about December 4, 2015, the employer did not ensure that employees were protected from the hazard of projectiles to the eyes in that neither the employee using a pneumatic nail gun nor the adjacent employee wore safety glasses.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    d-7 Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 556240, citation number 1, item number 1, and was affirmed as a final order on December 10, 2012, with respect to a workplace located at 14049 St. Paul Street, Thornton, Colorado.

29 CFR 1926.501(b)(11): Each employee(s) on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems:     (a)  On or about December 4, 2015, the employer did not protect employees from falls of 20 feet and greater while they were installing roofing materials on a roof without using fall arrest systems or other means of fall protection.
