29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      a) On or about December 11, 2015, employees were exposed to falls greater than 6 feet while they were installing metal roofing materials on the roof of a commercial building, without the benefit of a fall protection system being in place.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.       a). On or about December 11, 2015, employees were exposed to falls hazards created by the ladder they were utilizing to access the roof of a commercial building was placed in a area of high pedestrian traffic, without being secured to prevent accidental displacement nor were barricades placed around the ladder to keep traffic away.
