29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a. Autoneum North America, Inc. at the worksite located at 1150 N. Wynn Rd. in Oregon, Ohio: On or about December 17, 2015, the employer did not document and utilize energy control procedures for authorized employees who maintain and set-up the Scrim 6 machine. Employees setting up the machine by feeding fiber (scrim) to the table and maintenance employees who made adjustments to the machine's table height were exposed to thermal, pneumatic, and mechanical, energy sources.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  a. Autoneum North America, Inc. at the worksite located at 1150 N. Wynn Rd. in Oregon, Ohio: On or about December 10, 2015 the employer did not ensure energy control procedures for the Scrim 5 machine identified specific steps to follow to render safe the pneumatic and thermal energy sources.   Employees that maintained and set-up the machine were exposed to the moving parts of the gantry and burn injury from the heater bars.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    a. Autoneum North America, Inc. at the worksite located at 1150 N. Wynn Rd. in Oregon, Ohio: On December 17, 2015, the employer did not ensure employees who setup and/or maintain the Cell 5 and Cell 6 scrim machines were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that set-up or maintained the machine were not trained to isolate and/or render energy sources safe.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).    a. Autoneum North America, Inc. at the worksite located at 1150 N. Wynn Rd. in Oregon, Ohio: On or about December 17, 2015, employees were exposed to amputation, pinch point, and thermal hazards in that the Cell 6 Scrim machine was not locked out prior to setting up and servicing the machine.  Prior to accessing hazard areas of the machine for set up and maintenance activities such as, but not limited to, feeding a fiber roll and adjusting the table height, the employer did not implement energy control application steps as required by paragraph (d) of this section; the employer did not:    -  Ensure employees had the method or means to control hazardous energy.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  -  Verify that isolation and deenergization of the machine or equipment had been accomplished.    b. Autoneum North America, Inc.  at the worksite located at 1150 N. Wynn Rd. in Oregon, Ohio: On or about December 16, 2015, employees were exposed to amputation, pinch point, and thermal hazards in that the Cell 5 Scrim machine was not locked out prior to setting up and servicing the machine.  Prior to accessing hazard areas of the machine for set up and maintenance activities such as, but not limited to, feeding a fiber roll and adjusting the knife blade, the employer did not implement energy control application steps as required by paragraph (d) of this section; the employer did not:  -  Ensure employees had the method or means to control hazardous energy.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  -  Verify that isolation and deenergization of the machine or equipment had been accomplished.      Autoneum North America, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1063461, citation number 1, item number 2 and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 645 N. Lallendorf Rd., Oregon, OH 43616.
