Prior to accessing hazard areas of the machine for set up and maintenance activities such as, but not limited to, feeding a fiber roll and adjusting the table height, the employer did not implement energy control application steps as required by paragraph (d) of this section; the employer did not:    -  Ensure employees had the method or means to control hazardous energy.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  -  Verify that isolation and deenergization of the machine or equipment had been accomplished.    b. Autoneum North America, Inc.  at the worksite located at 1150 N. Wynn Rd. in Oregon, Ohio: On or about December 16, 2015, employees were exposed to amputation, pinch point, and thermal hazards in that the Cell 5 Scrim machine was not locked out prior to setting up and servicing the machine.  Prior to accessing hazard areas of the machine for set up and maintenance activities such as, but not limited to, feeding a fiber roll and adjusting the knife blade, the employer did not implement energy control application steps as required by paragraph (d) of this section; the employer did not:  -  Ensure employees had the method or means to control hazardous energy.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  -  Verify that isolation and deenergization of the machine or equipment had been accomplished.      Autoneum North America, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1063461, citation number 1, item number 2 and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 645 N. Lallendorf Rd., Oregon, OH 43616.
