29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: a) At the facility, on or about December 16, 2015 - A Komatsu FG20T-12, with an addition that affects the capacity and safe operations of the fork lifts did not have the manufacturer's approval prior to use, and maintenance instruction plates, tags or decals were not changed accordingly to reflect the use of the drum tipper. An employee uses a drum tipper attachment on the Komatsu FG20T-12 to lift 55-gallon drums. b) At the facility, on or about December 16, 2015 - A Hyster S50XM, with an addition that affects the capacity and safe operations of the fork lifts did not have the manufacturer's approval prior to use, and maintenance instruction plates, tags or decals were not changed accordingly to reflect the use of the drum tipper. An employee uses a drum tipper attachment on the Hyster S50XM to lift 55-gallon drums.
