29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) S&P Alliance of Northwest Ohio, Inc. on or about December 08, 2015, at the address of 25 Christopher Dr. Fostoria, OH: The employer failed to ensure an employee tasked with the service and maintenance of a commercial exhaust system atop of a commercial structures roof was protected from a fall. The employee working from this surface in this condition was exposed to an approximant 8 ft. fall to the ground level below.
