29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:      (a) A maintenance employee was trouble shooting a problem on the 200 KVA spot welder #5543, located on the south side of the machine shop. After the problem was identified, repairs were started without performing a lockout procedure on the machine and it cycled resulting in the amputation of the tips of two fingers.      ABATEMENT CERTIFICATION IS REQUIRED FOR THE ITEM, USE THE CERTIFICATION OF CORRECTIVE ACTION WORKSHEET FOR THIS PURPOSE.
