29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about 15 December 2015 and at times prior thereto, two workers were working from a towable boom lift without the use of a body belt with lanyard, exposing the workers to the hazard of falling approximately 20 feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about 15 December 2015 and at times prior thereto, two workers were performing roofing activities on a low slope, 4 to 12, roof without fall protection, exposing the workers to the hazard of falling approximately 20 feet, 8 inches to the ground below.
