OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that  employees were exposed to crushing hazards while working under automotive lifts.  Worksite Location: 1525 Central St, Leominster, MA;   On 12/11/15, an employee who had received no formal, documented lift training was injured when the vehicle he was dismantling fell off the lift that had not been inspected.       One feasible and acceptable means of abatement includes, but is not limited to, following the American National Standards Institute / Automotive Lift Institute Standard for Automotive Lifts- Safety Requirements for Operation, Inspection, and Maintenance, ANSI/ALI ALOIM-2008 to ensure: 1) Formal training is provided for all lift operators; including a review of the lift manufacturer's instructions; the vehicle manufacturer's instructions ; and the ANSI Lifting Guide  2) The lift operator training is documented, and 3) The lift safety features are inspected as required on a daily and monthly basis.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation and in addition, documentation that abatement is complete must be included with your certification.
