29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 16 December 2015 and at times prior thereto, employees without head protection were working below other employees performing roofing activities, exposing the employees to the hazard of being struck by falling or flying objects, materials and tools.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  On or about 16 December 2015 and at times prior thereto, the employer did not train employees on the recognition and avoidance of hazards to include, but not limited to, fall protection, ladder safety and struck by falling or flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 16 December 2015 and at times prior thereto, 4 workers were performing roofing activities on the second story roof of a home under construction without fall protection, exposing the workers to the hazard of falling approximately 21 feet 4 inches to the ground below.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about 16 December 2015, and at times prior thereto, three roofers accessed the roof of a residence under construction using an extension ladder that was not extended at least 3 feet above the upper landing and did not have a grasping device at the upper landing, exposing the roofers to a fall hazard of approximately 11 feet, 6 inches to the ground below.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  On or about 16 December 2015 and at times prior thereto, the employer failed to provide ladder training to 4 workers using a metal extension ladder to access the first story roof of a house under construction, exposing the workers to the hazard of falling approximately 11 feet 4 inches.
