29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     On or about January 28, 2016, and at times prior thereto and thereafter, employees were using corrosive chemicals to clean the facility without the use of safety goggles and/or face shields, exposing employees to the hazards of corrosive chemical splashes to the eyes.    a) Mark E II, one step disinfected germicidal detergent and deodorant containing decydimethyloctylammonium chloride and tetrasoduim ethylenediaminetetraacetate.   b) Action Floor Stripper containing monoethanolamine and sodium hydroxide.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.    On or about January 28, 2016, and at times prior thereto and thereafter, employees were using corrosive chemicals such as, but not limited to Mark E II, one step disinfectant germicidal detergent and deodorant to disinfect restrooms without the appropriate hand protection. The employer provided synthetic vinyl chloride gloves without neoprene, butyl rubber, or nitrile rubber for preventing chemical burns, exposing employees to the hazard of contact to the skin with corrosive chemical.  The light duty gloves were tearing during use and allowing chemicals through the material.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:     On or about January 28, 2016, and at times prior thereto and thereafter, employees did not receive effective information and training on the hazards of chemicals that could cause corrosive splash injury during opening packages, pouring, mixing and use.  Information and training was delivered to employees via video that gave a general description on how to read and follow directions found in the labels of containers. The training video did not specify the hazards of the chemicals employees use at their worksite and which specific measures to be taken in order for employees to protect themselves, exposing employees to inhalation, ingestion, and dermal hazards of chemicals.    a) Mark E II, one step disinfected germicidal detergent and deodorant containing decydimethyloctylammonium chloride and tetrasoduim ethylenediaminetetraacetate, which are corrosive to eyes and skin.       b) Action Floor Stripper containing monoethanolamine and sodium hydroxide which are corrosive eyes and skin.
