A, at the Mixer Operator level and across from the rest room: An employee was painting the new pipes and exposed a fall hazard at the  floor opening 13 deep X 10 long X 10 wide and exposed to a fall hazard down to the 1st floor from the 2nd floor because; the floor hole had not been filled in, covered or guarded.
