29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 12/15/15 located on the 2nd floor of Bldg. A, at the Mixer Operator level and across from the rest room: An employee was painting the new pipes and exposed a fall hazard at the  floor opening 13 deep X 10 long X 10 wide and exposed to a fall hazard down to the 1st floor from the 2nd floor because; the floor hole had not been filled in, covered or guarded.

29 CFR 1926.350(a)(9): Compressed gas cylinder(s) were not secured in an upright position:  On or about 12/15/15 located on the ground floor of the Mixing building NE corner outside Five Argo Compressed Acetylene cylinders were not stored nor secured in an upright position away from locations where they could be ran over by forklifts, pallet jacks and other moving equipment and employees.
