29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as clearing jams, setting up machines. On or about December 18, 2015 and at times prior to, employees were exposed to caught-in hazard where the employer did not develop lockout and tagout procedures for employees clearing lettuce from a roller on a conveyor belt, such as but not limited to Salad Trim Line 3. The Employer does not have a procedure.
