29 CFR 1926.52(d)(1): In all cases where the sound levels exceeded the values shown in Table D-2 of 29 CFR 1926.52, a continuing, effective hearing conservation program was not administered.  Location: Worksite  The employer's Hearing Conservation Program was inadequate because an employee was exposed to an 8-hour time-weighted average of 96.7 dBA, or a dose of 255.7%, while using a needle gun to chip brick and mortar inside a building  at Commonwealth Avenue, Boston, MA, and the employer had not instituted the following elements of a program:  1) Monitoring of employees' noise exposure 2) Institution, if feasible, of engineering, work practice, and administrative controls for excessive noise 3) Employee training and education regarding noise hazards and protective measures 4) Baseline and annual audiometry
