29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure:  Lab:   a) The hepatitis vaccination series was not offered to workers in the laboratory the where there was potential exposure to bloodborne pathogns when processing blood samples and urine samples with visible blood.  b) The hepatitis vaccination series was not offered to all of the phlebotomists where there is potential exposure to bloodborne pathogens during blood drawing operations.

29 CFR 1910.1030(g)(2)(vii): The employer's training program did not contain the minimum elements required by 29 CFR 1910.1030(g)(2)(vii)(A) through (g)(2)(vii)(N):  Laboratory: Annual bloodborne pathogen training, that included all the required training topics, was not provided to phlebotomists and laboratory workers where there was the potential for exposure to bloodborne pathogens.    NOTE: Ensure training includes instructions on 1) Safe sharp disposable methods of needle and blood tube assembly holders; 2) Using adequate size sharp containers for the quantity of work performed to avoid overfilling, and 3) How to access, at blood drawing locations, handwashing facilities that are immediately available in the event of an exposure incident. 4) How to decontaminate surfaces in the event of a blood spill at a blood drawing site.
