29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:        a) 146 Pearl Street, Providence, RI: On or about 12/16/2016, two employees were working adjacent to electrical power lines and had not been adequately trained in the recognition and avoidance of live overhead primary, secondary and house drop electrical lines of voltages up to 75KV.        b) Establishment: On or about 12/16/2016, supervisors had not been adequately trained in the recognition and avoidance of electrical hazards when they are assigning employees work which would expose them to overhead primary, secondary and house drop electrical lines with voltages up to 75KV.

29 CFR 1926.416(a)(1): An employee was permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means:        a)   146 Pearl Street, Providence, RI:  On or about 12/16/2016, two employees were permitted to perform carpentry work from within a Self-Propelled Boom Lift proximate to a 75KV overhead primary and secondary lines located approximately 32 feet above the sidewalk that were not de-energized and grounded or effectively guarded.

29 CFR 1926.416(a)(3): Before work is begun, the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists and did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken:        a) 146 Pearl Street, Providence, RI:  On or about, 12/16/2016, the employer did not ascertain whether any part of an energized overhead electric power circuit was so located that the performance of work would bring employees or the TEREX Self-Propelled Boom Lift and/or other equipment, into physical or electrical contact when employees were assigned carpentry work adjacent to primary and secondary live overhead lines. The employer also did not post and maintain proper warning signs nor advise the two employees of the location of the primary and secondary live overhead electrical power lines, the electrical hazards of those lines and the protective measures to be taken.
