29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Storage Mezzanine in Warehouse - Employees climbed onto and worked from the storage mezzanine, 10 feet above the floor, in order to get items out of storage or to place items into storage. While working from the storage area to retrieve or stow items, employees did not use personal fall arrest or restraint equipment to protect them from falling to the floor below from the open side, which extended the width of the warehouse, on or about September 1, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.176(b): Storage of material created a hazard: (a) Warehouse, D4 Location - The employer did not have any of the four columns of the steel storage rack, used for storing materials, anchored to the floor, exposing employees to a struck-by hazard from collapse or tipping of the rack, on or about December 21, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state that employees and their representatives were informed of this abatement.
