29 CFR 1910.269(a)(2)(ii)(D): The proper use of the special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools for working on or near exposed energized parts of electric equipment. On or about 12/17/15, at the location of 912 E. 55th St. in Wichita, KS, employees in an articulating boom engaged in the replacement of dead end bells (conductors) with the use of a chain hoist were exposed to electrical shock hazards. The employees were exposed in that a complete blanket of insulation and shielding material was not used over the 12kV power line.

29 CFR 1910.269(l)(3)(iii): The employer did not ensure that no employee approached or took any conductive object closer to exposed energized parts than the employer's established minimum approach distance, unless provisions described in (l) (3) (iii) (A) - (C) were followed: On or about 12/17/15, at the location of 912 E. 55th St. in Wichita, Ks., employees in an articulating boom engaged in the replacement of dead end bells (conductors) with the use of a chain hoist were exposed to electrical shock hazards. The employees were exposed in that the conductive metal chain hoist was not prevented from contacting an energized 12kV power line.
